Job Summary:
Under the general supervision of the Senior Social worker ; the Junior Social Worker is responsible for the counseling clients under supervision of the social worker or senior social worker at the rehabilitation center.
SPECIFIC DUTIES AND RESPONSIBILITIES:
Attending clinics in rehabilitation centers, Mobile Outreach and CBR projects
Assessing and certifying free service and or cost sharing clients
Assessing and offering advice or counseling clients under supervision of senior or Class two social workers.
Preparing individual service plan for every child admitted to the center
Facilitate admissions and discharges and follow-up care
Report changes in patient condition / treatment to supervising physiotherapist
Collaborate with the rehabilitation team regarding assessment, treatment implementation, outcome and discharge
Record and maintain the patient information in the clinical file
Maintain a high level of confidentiality and privacy of privileged information
Demonstrate empathy and compassion for all patients, regardless of health, age, race, ability ,disability or culture
Involve in awareness, raising training, community, education services.
Taking picture of every child and writing case histories admitted following the progress attained
Writing reports on special events held at the center or project or visits made.
Conduct any related work as requested by his supervisor
