GOAL is a non-for profit organization that has been working in Ethiopia for over 30 years, towards ensuring the poorest and most vulnerable in our world and those affected by humanitarian crises have access to the fundamental rights of life, including but not limited to adequate shelter, food and livelihoods, water and sanitation, nutrition, healthcare and education. GOAL implements a range of multi-sectoral development, resilience, recovery and humanitarian responses programmes by being sensitive to cross-cutting issues including gender, accountably, child protection, HIV/AIDS. Currently, we work with pastoralist, agropastoral, pastoral dropouts, rural small holder farmers, rural/urban youth and street children to respond to humanitarian crises and build resilience in Ethiopia working closely with local communities, local government, donors and other key actors. GOAL E Humanitarian Response is centered on nutrition, WASH, livelihoods, multipurpose cash, NFIs and protection interventions in country wide while the recovery and development focus on livelihoods, economic opportunities, health, health and nutrition system strengthening and WASH with solid emphasis on integrated programming and community-based approach in Oromiya regional state. The Street Children programme is implemented with local partners in Addis Ababa and Hawassa.

GENERAL DESCRIPTION OF THE ROLE

  • The primary objective of the Referrals Nurse position is to facilitate the efficient and effective referral of patients to appropriate healthcare facilities or specialists for further evaluation, treatment, or management. The Referrals Nurse will work closely with selected Health facilities for service provider, healthcare providers, patients, and external healthcare facilities to ensure timely and coordinated referrals

KEY AREAS OF ACCOUNTABILITY/DUTIES

  • Coordinate referrals for patients requiring specialized services, diagnostic tests, or consultations with external healthcare facilities or specialists.

  • Maintain accurate and complete documentation of referral requests, approvals, and outcomes in the patient's medical records and referral tracking system.

  • Receive referrals related to vulnerable individuals raising issues in accessing health care and provide them with the best opportunity to connect/seek timely health care services at health facilities.

  • Communicate effectively with patients, families, and healthcare providers regarding the need for referral, referral process, and follow-up care instructions.

  • Follow up with patients to ensure they have received the necessary services and support after referral, including scheduling appointments, obtaining test results, and addressing any barriers to care.

  • Collaborate with internal healthcare teams, including physicians, nurses, and other departments, to ensure seamless coordination of care and continuity of treatment.

  • Establish and maintain relationships with external healthcare facilities, specialists, and community resources to facilitate timely and appropriate referrals.

  • Provide needed assistance to the targeted beneficiaries and make necessary and timely referrals to primary health care centers and to specialized health care services (e.g., referral to hospitals including psychiatric admission, admission due to severe malnutrition, emergency obstetric care, and others) and referral to other assistance for non-Health related services whenever needed, respecting confidentiality and the right of the beneficiary/patient who need medical care.

  • Follow up on the referred cases to ensure their access to the services needed.

  • Provide informal one to one awareness sessions to referred beneficiaries on several topics if needed, such as, but not limited to, non-communicable diseases, IYCF, and waterborne diseases and hygiene.

  • Ensure timely reporting of the activities to the supervisor and by using RAIS for individual cases, according to the health counseling lines, and filling out the excel sheet for the reporting/follow up of environmental health events.

  • Raise PR for the patient accommodation and flight or facilitate transportation 

  • Perform other duties as required or assigned.

Job Requirements

Bachelor’s in science of nursing/Health officer and COC, and registered license from the recognized institution and 4 and above years previous experience in recognized private and Governmental specialized, and referral hospitals. Or working NGO.
