Job Description

The case manager creates and strengthens relationships and networks with respective donor staff, Government stakeholders, and ART clients to provide adherence counseling and help the ART clients get integrated clinical services. The case manager reports to the health care providers
Synergizes the team roles and responsibilities of the specific clinic and BPDO at large,
Provides standardized adherence counselling to chronic HIV care services,
Provides counselling on readiness for ART initiation,
Remind all VL-eligible clients and facilitate at the community level VL sample collection
Provide post-test counseling and initiate ART service
Assure the client of the confidentiality of the counseling process
Provide information on condoms, including condom demonstration, promotion & provision
Identify health facilities in the specific town that can provide a MARPs-friendly service and build a relationship and feedback mechanism with them.
Design a tracing mechanism for ART clients lost from the service
Record all ART clients on the appointment calendar and remind them before the appointment date
Provide counseling according to the individual’s needs. Risk behaviors can vary from individual to individual, and the way you approach will also vary accordingly.

Person specification:
Minimum Grade 10 completed, 3 years’ experience in community facility service
NGO experience is more advantageous in shortlisting
Age is not more than 30 years
Committed to addressing the community
Competencies:
Good interpersonal skills and a team player with sensitivity to cultural diversity
Ability to work under pressure
Ability to speak a local language is advantageous
